Practice Support
Practice support exists to give primary care physicians and their teams the extra resources they need to improve the care they deliver. Under the patient centered medical home programme, that support covers two connected things: care coordination and practice transformation.

Care coordination payments
Care coordination payments are paid quarterly on a per-beneficiary-per-month basis for attributed patients. The payments are not a bonus for a single good result; they are intended to support the ongoing operational costs that come with coordinating care, including the tools, technology and services a practice needs as it changes how it works.
Because the payments are recurring, a practice can plan around them rather than depending on a one-off sum. That matters for change that takes time, such as redesigning how the front desk, nursing staff and physicians share information about a patient's care.
Practice transformation
Enrolled practices are eligible to receive practice transformation support from the Department of Human Services through a vendor, the Arkansas Foundation for Medical Care. The vendor works with practices on the practical details of becoming a medical home, from workflow redesign to preparing for the requirements the programme measures.
Practices that want to know more about the services the transformation vendor provides can contact the foundation by email at the address given in the programme's contact information. The foundation also provides a range of general support to Arkansas providers beyond the transformation programme.
Where to look for more detail
The PCMH manual and the programme's frequently asked questions are the two reference points for the detail of practice support: how the payments work, what the transformation vendor offers and what a practice is expected to do to keep its support.
Because support is tied to participation, a practice should treat the manual as the description of record and use the questions section for the points that arise in day-to-day operation. The programme's contact channels handle queries that the written material does not settle.
Support alongside reporting
Practice support does not stand alone. It sits next to the reporting that shows a practice how it is performing and the shared savings payments that reward results. Support helps a practice make the change, reporting shows whether the change worked, and shared savings returns part of the benefit when it does.
That combination is what the programme intends. Assistance without a way to measure progress would leave practices without direction, and rewards without support would leave them without the means to earn them.
In practical terms, transformation support can involve looking at how a practice handles a whole patient journey, from the first contact to follow-up, and finding the points where information is lost or duplicated. A coach may help a team map the workflow, agree who is responsible for each step and set a small number of changes to try. The point is not to impose a single way of working but to help the practice find one that fits its patients and can be sustained after the support period ends.
The support also recognises that transformation competes with the ordinary pressures of running a practice. A small team cannot always spare a clinician for a redesign project, which is why the coordination payments are structured as recurring support rather than as a grant for a single exercise. The funding is meant to make the improvement work affordable to sustain, not merely to start it.
What practice support includes
- Care coordination payments made quarterly.
- Payments calculated on a per-beneficiary-per-month basis.
- Support for coordination tools, technology and services.
- Practice transformation assistance from the state's vendor.
- Guidance through the PCMH manual and the programme's questions section.
- Programme contact channels for queries not covered in writing.

Support arrangements, payment methods and vendor details are set by the programme and are reviewed over time. Practices should confirm the current position in the official programme materials before relying on any particular arrangement.
